How do I cancel my subscription?

To cancel your subscription to ChartMySelf, go to your Account Settings and choose Cancel Subscription.

How does ChartMySelf relate to my medical records?

ChartMySelf is a daily personal self-assessment tool that allows private monitoring of health and lifestyle. ChartMySelf is not, in any form, intended to replace your official medical record, health record, or medical chart. You are responsible for the process of adding any charts created with ChartMySelf to your medical records or personal health records if you choose to do so. You are also responsible for sharing charts created with ChartMySelf with your health care provider if you choose to do so.

When does my ChartMySelf session log out?

Your ChartMySelf session will automatically log you out after 30 minutes of inactivity. This increases security of your account and data.
